Yes, waivers must be completed in full for all carriers. However, carriers may have differing stances on whether the employee needs to provide proof they have a valid waiver reason.
Anthem Blue Cross is B&P’s only carrier that requires ID cards to prove participation.
The following carriers reserve the right request ID cards to prove participation:
All other carriers will accept the reason listed on the waiver.
