Two (2) new plans excluding coverage for contraceptives will be available for Small Gruop employers meeting the criteria for religious exemption to the Patient Protection and Affordable Care Act (PPACA) - Women's Preventive Care Mandate. These two new plans are the same as the current Classic $30 HMO and Solution PPO 3500 but do not provide coverage for contraceptives or permanent sterlization. These plans are not available in any Anthem Blue Cross portfolio. They may only be offered standalone or as dual choice. Employer groups who choose these plans must meet the criteria below and complete a certification form.
