Network: UHC Alliance Contract with Affinity Medical Group Expires January 2016

UHC would like to inform you that the portion of their agreement with Affinity Medical Group specific to their Alliance HMO product only will be terminating effective December 31, 2015.

Please note that while the UnitedHealthcare of California and Affinity Medical Group are in negotiations for the full commercial network, UHC believes that Affinity will remain contracted with UnitedHealthcare for their full network commercial HMO and Medicare Advantage HMO products as of January 1, 2016.

Member Notification
Effective January 1, 2016, Affinity Medical Group will no longer be considered a network provider for UHC's Alliance HMO members. This is due to a lack of membership growth for this particular product in the Oakland area.

Per regulatory and contractual obligations, affected members (those who are assigned to Affinity Medical Group) will be notified in advance of a contract's pending expiration date. Timely notification of a potential contract termination can help members plan accordingly. Once the contract expires, Affinity Medical Group will no longer be part of the Alliance HMO network, and members are advised as follows:

UHC regrets any inconvenience the Affinity Medical Group termination for their Alliance HMO product may cause.
