Employee eKit

The MetLife Employee eKit provides you access to the various forms and resources, via the links below, to assist you with your enrollment. Please review the Guidelines section for the pertinent details.

Once you have your enrollment, please return the completed forms and applications to your company's Benefit Administrator.

GUIDELINES

STEP 1:
Open, print and read the Employee Enrollment Form.

STEP 2:
You may choose between the plans offered by your employer. Please complete the Employee Enrollment Form for the plan you wish to enroll.

  • Please discuss any questions you may have with your Benefit Administrator.

STEP 3:
Turn in the completed application to your company's Benefit Administrator.

  • Once your company's benefit plan has been approved, your employer will receive your certificate of coverage and benefit card(s).
