Anthem Metallic Dental rates have been reduced by 10% in rating area 1 of Northern California for April 1, 2018 effective dates.
Rating Area 1 is three-digit zip codes 940, 941, 943-951, and 954; all other three-digit zip codes are considered Rating Area 2 for rating purposes.
These zips include some areas in the following counties; however, some zips within these counties may not be included in Rating Area 1.
