Anthem's 2021 Site of Service Program Help Members Save

With Anthem's Site of Service program for 2021 small groups, employees can save hundreds, or even thousands, of dollars when they seek care from lower-cost independent providers listed on Anthem's Find Care tool at anthem.com/ca.

How to use Site of Service

Employees can log on to anthem.com/ca, search for providers for the following services and pay less when they use independent providers:

Lab tests

All in-network independent labs are part of the Site of Service program. On the Find Care tool, under Specialty, employees can search for “Independent Laboratory” in their zip code or county. For non-health savings account (HSA) plans, employees pay $0 and their deductible is waived. Employees on HSA plans must meet their deductible before $0 copay applies.

Radiology and advanced diagnostic imaging Employees can use an independent radiology center for X-rays, ultrasounds, MRIs, or CT scans. On the Find Care tool, under Specialty, employees can search for “Radiology” “Clinic/Center” in their zip code or county.

Outpatient surgery

For procedures performed in an Ambulatory Surgical Center (ASC), copays are lower than when the same procedure is performed at a hospital. Preferred provider organization (PPO) and HSA plans have an additional copay for outpatient hospital procedures that is waived when employees use an ASC. On the Find Care tool, under Specialty, employees can search for “Ambulatory Surgical Center” in their zip code or county.

Freestanding providers offer cost savings

The chart below shows average costs for members on PPO, exclusive provider organization (EPO), and HSA plans, by service and by setting. Allowed amounts will vary depending on services billed, provider, and plan benefits. If employees need additional care as part of the procedure, they may have to meet a deductible and/or pay coinsurance.

This chart shows average costs for members on the Anthem Gold HMO 30 plan. This plan requires referral from a primary care doctor for services. Employees should speak with their doctors about Anthem’s Site of Service program to discuss referral to the most cost-efficient facility.
