CaliforniaChoice Plan & Rate Updates Effective September 2024

CaliforniaChoice is intrducing a new Anthem Blue Cross plan effective September 1, 2024. With this update, they will also be updating their forms and marketing materials. Please note that you must use the correct applications beginning September 1, 2024. CaliforniaChoice will not accept write-ins or alternations.

Vivity Added to Portfolio

CaliforniaChoice is adding a new Anthem Blue Cross plan on the Vivity network:

• Anthem Blue Cross Platinum Vivity HMO B

• $0 Individual Deductible
• $3,350 Individual Out-of-pocket Maximum
• $20 Primary Care Physician visit
• $40 Specialist visit
• $250/day, 4 days max/admit Inpatient
• $5/$15 Tier 1 Rx
• $25/$35 Tier 1 Rx
• $75/$85 Tier 1 Rx
