ChoiceBuilder is not making any changes to their plans or rates.

Plan Offering Updates

ChoiceBuilder is adding MetLife employer-sponsored Life insurance effective December 1, 2022.

MetLife Life Insurance Guidelines

• Employer sponsored Life can be added at initial enrollment or at renewal with no reduction in Guarantee Issue amounts (not available to add off-cycle)
• Employer must select only one carrier to offer employer sponsored Life to their employees
• Employer required contribution for employer sponsored Life benefits is 100%
• Employee participation required for employer sponsored Life benefits is 100%
• All active full-time employees working 20 or more hours per week are eligible for coverage

Updated Materials & Applications

ChoiceBuilder will be updating marketing materials and enrollment applications to reflect these updates. Only updated applications and change request forms for December 1, 2022 will be accepted beginning with the December 1, 2022 effective date. Write-ins or alterations are not accepted.
