ChoiceBuilder Plan & Rate Updates Effective January 2024

ChoiceBuilder announced plan and rate updates for January 1, 2024 effective dates. All rates are available for quoting!

Rates & Benefits

Rates

• Anthem Blue Cross rates will change.
• All other rates will remain the same for all other ChoiceBuilder plans.

Benefits

• EyeMed: Standard Progressive Lenses has changed from $65 copay to Covered in Full.
• VSP:
  • Essential Medical Eye Care and VSP LightCare venefits included at no additional cost.
  • Platinum:
    • Frame benefit has changed from $180 Allowance to $250 Allowance
    • Contact Lenses benefit has changed from $150 Allowance to $200 Allowance
  • Gold
    • Frame benefit has changed from $180 Allowance to $200 Allowance
    • Contact Lenses benefit has changed from $150 Allowance to $180 Allowance
• All other benefits will remain the same for all other ChoiceBuilder plans.

Forms

Only the updated applications for January 1, 2024 will be accepted beginning with the January 1, 2024 effective date. Earlier versions will not be accepted. Write-ins or cross-outs are not accepted.
