ChoiceBuilder announced plan and rate updates for January 1, 2025 effective dates. All rates are available for quoting!
Rate Updates
Rates are only changing for the Platinum plans. There are no rate changes effective January 1, 2025.
New Plans Effective January 2025
ChoiceBuilder is introducing a new Ameritas Platinum Plus PPO Dental plan. This plan includes:
- $25 Lifetime deductible
- $3,000 Annual maximum
- Optional Orthodontia
- 80th U & C for Out-of-Network benefits

Benefit Updates
Benefits will be changing for existing Ameritas, Delta Dental, and MetLife Dental plans. MetLife will also be adding an EAP to their Life Plans. Additionally, Landmark Healthplan Chiropractic/Acupuncture benefits will change effective March 1, 2025.
Dental
Ameritas: Employer Sponsored and Voluntary Plans
- The Orthodontic Takeover Credit has changed from “At initial group enrollment employer sponsored groups with 10+ eligible employees and prior continuous uninterrupted orthodontic coverage of 12 months, will waive orthodontic waiting period” to “At initial group enrollment employer sponsored groups with 5+ eligible employees and prior continuous uninterrupted orthodontic coverage of 12 months, will waive orthodontic waiting period.”
- The Pre-existing Conditions (missing tooth) benefit has changed from “Not Covered” to “Covered”
Delta Dental: Employer Sponsored and Voluntary Plans
- The Congenital Missing Tooth benefit has changed from “Not Covered” to “Covered” Silver PPO (Voluntary only)
- The Major and Orthodontic waiting periods have been changed from “12 Months” to “None” for (IN & OON)
MetLife: Employer Sponsored and Voluntary Plans
- The Orthodontic Take Over Credit has changed from “Does Not Apply” to “Automatic” Platinum Plus PPO (Employer Sponsored only)
- The Annual Maximum benefit has changed from “$2,500” to “$5,000” for (IN) and from “$2,000” to “$2,500” for (OON)
Life
MetLife: Employer Sponsored Plan
- Employee Assistance Program (EAP), provided through TELUS Health, is now included at no additional cost
Landmark Health Plan Updates Effective March 1, 2025
The following changes will go into effect on March 1, 2025, for Landmark Healthplan Employer-Sponsored and Voluntary Plans. These changes will apply to new groups and inforce business on March 1, 2025. This changes applies to all groups on March 1, 2025, regardless of effective date. Landmark Healthplan benefits will be changing as follows:
- The New Patient Evaluation & Management benefit will change from “$65 per visit” to “$80 per visit
- The following benefits will change from “$50 per visit” to a “$65 per visit”:
- Established Patient Re-Examination & Management
- Modalities
- Therapeutic Procedures
- Chronic Manipulative Treatment
- Special Services
- Chiropractic Fee Schedules:
- Fee Schedule A will be changing from $65 for new patient initial visits/$50 for recurring visits to $80 for new patient initial visits/$65 for recurring visits
- Fee Schedule B will be changing from $60 for new patient initial visits/$40 for recurring visits to $75 for new patient initial visits/$55 for recurring visits

Forms
Please use the applications dated for January 1, 2025 as new plans are available. Earlier versions will not be accepted. Write-ins or cross-outs are not accepted.
