Health Net and Prime Care Medical Group of Chino Valley Contract Update

On February 28th, Health Net notified brokers that PrimeCare Medical Group of Chino Valley (PPG # 741/1252) will no longer be contracted with Health Net, effective April 30, 2023, and will no longer be a part of the following Health Net networks across their Commercial lines of business:

• WholeCare
• UC Blue & Gold HMO
• ExcelCare
• POS
• Full Network HMO
• SmartCare HMO
• Ambetter HMO

Since the February 28th communication, there have been some updates, which Health Net wanted to make you aware of. 

• The termination has now been delayed to become effective 6/1/2023.

Member Transition 

Health Net will notify their impacted members via mail at the end of March advising them that effective June 1, 2023, their medical group will no longer participate as a Health Net network provider. Members will be reassigned to a new PPG and/or PCP if their PCP does not participate with any other Health Net contracted PPGs.  

Members who want to select a Primary Care Physician or medical group other than the one selected for them may call or e-mail Health Net to do so. 

Members currently receiving care or who are in treatment for ongoing, chronic conditions, will be referred to Health Net's Transition of Care department to ensure uninterrupted access to care. Members can call the Customer Contact Center for assistance or can complete a Transition of Care form.

Members will receive a new ID card with their new IPA/Medical Group and/or PCP with a June 1, 2023 effective date.
