Health Net's CommunityCare HMO plans have all the advantages of an HMO - plus extra flexibility for specialist care. Here's a quick overview.
CommunityCare HMO is offered by Health Net of California, Inc. It lets small business groups in L.A., Orange and San Diego counties offer a Gold, Silver or Bronze level plan - and save! Employees gain the benefits that put health first. And they have more choice because CommunityCare HMOs have fewer restrictions for specialist care than other HMOs.
First, members choose:
- A primary care physician (PCP) from a contracted medical group within CommunityCare's Tailored Network, or
- A PCP from Health Net's Direct Network of Southern California to coordinate their care.
(Note: Members will receive an ID card with their selected PCP's name and either the name of the medical group or the Direct Network affiliated with the chosen PCP.)
Then:
- Their PCP can refer to any specialist in the CommunityCare Network. Members aren't restricted to specialists within the PCP's participating provider group (PPG).
- Members can seek care at any urgent care facility in the CommunityCare Network - even those outside their PPG.
