Information on Form 1095-B Health Coverage Statements for Health Net of California, Inc. and Health Net Life Insurance Company (Health Net)
Health Net will mail Form 1095-B (statement of health care) for the 2018 tax year to subscribers' home addresses between February 11, 2019 and February 15, 2019. The form will be sent to subscribers who were enrolled in the following plans:
- Individual & Family Plans, off-exchange
- Small Business Group
- Covered California for Small Business
- Large Business Group
