- Provide a range of annual maximums from $500 to unlimited – truly allowing for flexibility per group need

*Please note: The new Dental options will be available in phases. Phase 1 will be available for a 3/1/2022 quote date and a 4/1/2022 effective date. Phase 2 will be available in Q3 of 2022, and Phase 3 is TBD. Phase 1, available to quote now, includes the following 13 states: AR, AZ, CA, CT, DC, ID, IL, NJ, PA, SC, TX, UT, and WI. Phase 2, available in Q3 of 2022, includes the following 10 states: CO, FL, GA, KY, LA, MD, MO, NC, OH, and VA. Phase 3, availability TBD, will include all remaining states.
