Network: Verity Discontinuing Services on July 1, 2019

Verity, which includes All Care Medical Group, Allied Healthcare Providers and Verity Medical Foundation (collectively "Verity") will discontinue all services on July 1, 2019.
Anthem Blue Cross will be mailing letters to subscribers currently enrolled with Verity informing them that they will be transferred to alternate participating medical groups. Letters are also being sent to agents informing them of the situation. For your reference, below are copies of these letters.
