UHC Plan & Rate Updates Effective January 2025

UHC announced plan and rate updates for January 1, 2025 effective dates. Rates are available for quoting!

Rates

Quarterly medical changes (averages, may vary by plan and region):

• HMO: -2.4%
• PPO:
  • Northern CA: 0.3%
  • Southern CA: 0.5%
• Navigate:*
  • Northern CA: 7.9%
  • Southern CA: 5.7%

Annual medical changes (averages, may vary by plan and region):

• HMO: 0%
• PPO:
  • Northern CA: 5.9%
  • Southern CA: 4.6%
• Navigate:*
  • Northern CA: 7.9%
  • Southern CA: 5.7%

*Navigate plans last had a rate change effective January 1, 2024, so the Navigate quarterly and annual rate changes are the same for January 1, 2025.

Portfolio Updates

UHC is making the following changes to their portfolio for the 2025 plan year:

• 57 plans will be offered in the Choice Simplified Package and 12 plans in the MultiChoice State Package.
  • The Silver PPO Non-Differential will remain available in both packages as the OOA plan when Select Plus is not available.
• The "Care Cash" benefit is being removed from four plans in 2025, these plans will still include either the Core or Premium Rewards depending on the plan:
  • Select Plus Silver 55/1950/60% Care Cash & Core Rewards (DI-AG) maps to Select Plus Silver 60/1950/60% Core Rewards (DZ-HT)
  • Core Silver 55/1950/60% Care Cash & Core Rewards (DH-95) maps to Core Silver 60/1950/60% Core Rewards (DZ-HI)
  • Select Plus Bronze 7500/50% Care Cash & Premium Rewards (DH-9Q) maps to Select Plus Bronze 7500/50% Premium Rewards (DZ-G2)
  • Core Bronze 7500/50% Care Cash & Premium Rewards (DH-9P) maps to Core Bronze 7500/50% Premium Rewards (DZ-GZ)
• $0 cost share for virtual visits on all plans except for HDHP plans.
• The infertility optional benefit will continue to be offered with a 3.4% load for HMO plans and 4.9% increase for PPO plans.
• The following two plans are not Creditable to Medicare Part D, all other plans are creditable:
  • Select Plus Bronze 7500/50% Premium Rewards (DZ-G2)
  • Core Bronze 7500/50% Premium Rewards (DZ-GZ)
• UHC's Rx Network is the Broad Rx Network across all plans (National network).
  • 2025 PDLs will be posted to Prescription Drug Lists | UnitedHealthcare (uhc.com) once available. The following PDLs apply based on network:
    • Under the California plans category, Small Group - Standard plans section, select the following:
      • Navigate plans = Standard Tier 4 PPO Prescription Drug List CDI Version
      • All other plans = Standard Tier 4 HMO and PPO Prescription Drug List DMHC Version
