1-800 Contacts, the popular online provider, is in the UHC Vision network. Based in Draper, UT, 1-800 Contact is the largest contact lens supplier in the U.S. They carry brands including Johnson & Johnson, Bausch & Lomb and CooperVision.
Eligible members can apply their contact lens allowance by logging in at 1800contacts.com and selecting the insurance link.
