Plan Name Reminders
- PPO A changed to PPO Value.
- PPO B12 changed to PPO Enhanced.
- PPO C changed to PPO Plus Premier Enhanced.
- PPO Plus Premier Value (accesses Premier network) was introduced in 2016.
Dental Available to Two (2) Plus Enrolled
- PPO Classic Plans: Minimum 2 enrolled and 75% participation.
- PPO Options Plans (50-99 Eligibles): Minimum 35 enrolled and 75% participation.
- DeltaCare USA Plans: Minimum 2 enrolled.
Calendar Year Maximums
- 2-4 Enrolled (PPO Classic Plans): $1,000 and $1,500 Calendar Year Maximums available.
- 5-99 Enrolled (PPO Classic Plans): $1,000, $1,500 and $2,000 Calendar Year Maximums available.
- 50-99 Eligibles (PPO Options Plans): $1,000, $1,500 and $2,000 Calendar Year Maximums available.
Orthodontia
- PPO Classic Plans: Minimum 10 enrolled; Child only coverage offers $1000 and $1500 benefit levels.
- PPO Options Plans: Minimum 35 enrolled; Adult & Child coverage offers $1000 and $1500 benefit levels.
- DeltaCare USA Plans: Minimum 5 enrolled; Adult & Child coverage.
EyeMed Vision
- Enhanced and Standard Plans: 5-99 eligibles; enrollment must match Dental.
- 24-month contracts for new business with January 1, 2017 through December 1, 2018 effective dates.
