bpQuote Sample Reports

Group Information

Cover Page	Generic quote cover page that may be customized with the broker's logo.
Census (EEs Only)	Also known as the Group Profile, this report includes the company and broker name, effective date and basic employee census data.
Census (EEs + Dep)	This report includes the company and broker name, effective date and basic employee and dependent census data.

Selected Medical Plan Information

Plan Grid	A simple plan summary with group totals displayed vertically. Illustrates number of employees included in total with no employer contributions.
Side by Side	This is the most popular report. It summarizes benefits and total premium for up to seven (7) plans per page.
Side by Side Detail	This report includes five (5) additional benefit categories plus premiums that the Side by Side does not include. Due to the extra details you may only include three (3) plans per page.
Employee Rate Side by Side	Similar to the Side by Side Detail, except it displays each employee's individual and dependent rate and the group total.
Employee Rate Detail	Similar to the Single Plan Detail report except benefits are displayed horizontally with an employee rate breakdown. Will accommodate two (2) plans per page if group size is not too large.
Member Rate Detail	This report provides a short benefit summary and an individual rate per each enrolling member in the displayed plan.
Full Benefits	The most detailed benefit summary in bpQuote; one plan per page.
Single Plan Detail	This report displays a summary of benefits, premium and contributions while also providing a rate breakdown by employee.
Underwriting Guidelines	Detailed underwriting rules by carrier.
Rider Report	This report displays the additional cost of riders by employee. Riders include: Pediatric Dental (Anthem and Blue Shield), Chiropractic, and Acupuncture.
Portfolio Grid	A summary of each carrier's portfolio from which the quoted plans are included.

General Plan Information

Current / Renewal	Displays the current plan's benefit summary and employee totals for both current and renewal plans.
Cost Comparison	Premium totals are broken down by employee, dependent and total in a vertical format. Easily analyze plans similar or less expensive than current plans and/or show all carriers and all plans in few pages.
Employee ACA Worksheet	Illustrates an employee's individual and each dependent's cost for each plan being made available for enrollment. It displays costs based on employer contribution, contribution period and can also factor in rollover contributions.
Employee Basic Worksheet	Illustrates an employee's individual and spousal cost for each plan being made available for enrollment.
Table Rates ACA	Also referred to as Book Rates. This report displays plan rates for every age category by rating area. Table Rates are also accessible from the Main Menu of bpQuote.

Dual / Multi Option Proposals

Dual Option	This report displays an employee rate breakdown of actual enrollment across one (1) HMO and one (1) PPO from the same carrier in one column; displays four (4) options per page.
Multi Option	Also known as the Multi Choice Plan Summary, this report displays total cost for employees and employer (in one column) when the group is offering multiple carriers and/or multiple plans.
Multi Option Final Rates	This report displays the final rate breakdown for the employees and employer including the total cost of plans and contribution costs when the group is offering multiple carriers and/or multiple plans.

Dental Plan Information

Side by Side Comparison	This report displays a summary of benefits and total monthly premium for four plans per page. An employee breakdown is listed on the same page if the size of the group is not too large.
Dental Plan Detail	This report displays a summary of benefits, premium and contributions while also providing a rate breakdown by employee.
Dental Cost Comparison	Easily analyze plans similar or less expensive than current plans and/or show all carriers and all plans in few pages.
Dental Employee Elect	Also known as an employee worksheet, it illustrates an employee's individual and tiered cost for each plan being made available for enrollment. It displays costs based on employer contribution, contribution period.
Dental Underwriting	Underwriting rules by carrier.

Life and Vision Plan Information

Group Life Proposal	Includes Carrier name, policy amount, total monthly premium and a breakdown of each employee's name, age, benefit and premium.
Group Vision Proposal	Includes in- and out-of-network benefit descriptions, total monthly premium and a breakdown of each employee's name, age. It also includes employee, dependent and total premium.
Vision Worksheet	Illustrates the vision costs for each employee.
